[1] Take a new file of 500 pixels, 500 pixels, of resolution 72 dpi in the RGB mode.
[2] Take the "Shape Tool" selete this one.
[3] Create a shape color is #7db605.
[4] Open a new layer then create the following shape with the help of ‘Pen Tool’ fill that layer with white color.
[5] Then Cntl+Click the 1st shape layer to get selection then press Cntl+Shift+I now hit the delete key.
[6] And then set the Opacity by 12%.
[7] Create a new layer [this layer should above background layer] now create the following selection
with ‘Elliptical Marquee Tool’ then go to Select>Modify>Feather Selection by 10px.
[8] Now fill the selection with #dcdcdc.
[9] Now create another selection with ‘Elliptical Marquee Tool’ then go to Select>Modify>Feather Selection by 4px.
[10] Fill that layer with #bbbbbb.
